In a world where aviation technology has reached astonishing heights, automation is being used to fly and land airliners, business jets, and is now being adopted into light aircraft. GPS systems can pinpoint an aircraft to within a few feet, and weather in the cockpit is eliminating most, but not all, surprises. So why do we keep crashing?

Human error is involved in approximately 65 to 95 percent of all aircraft accidents across both commercial and general aviation. These aren’t typically mechanical failures or manufacturing defects. Instead, they stem from overconfidence, fatigue, poor judgment, distraction, or a deadly blend of all the above. Landsberg, retired Vice Chairman of the NTSB and past President of the AOPA Air Safety Institute, outlines how even pilots with thousands of hours in the air fall victim to completely avoidable mistakes.
One of the most persistent psychological traps is overconfidence. The book recalls chilling incidents of pilots with extensive flight hours who ignored basic procedures. For instance, they believed their experience would compensate for deteriorating weather or low fuel. Overconfidence is a slow drift away from best practices, reinforced by the false security of “getting away with it” in the past. Pilots who cut corners and don’t face immediate consequences may start believing their luck or skill is enough. As Landsberg puts it, “Survivors study their mistakes, and those of others. But crash-prone pilots may not be so introspective until a millisecond before eternity.”
In several case studies, the pursuit of liability leads, or attempts to distort, where the responsibility of safe flight lies. Landsberg points out that the Pilot-in-Command is the final authority for the safety of flight and likewise, has the most to lose. The efforts to shift the blame are done for economic purposes, as opposed to getting to the root cause for prevention.
Then there’s optimism bias. It is the dangerous belief that “it won’t happen to me.” This attitude leads pilots to launch into deteriorating weather, stretch fuel limits, or press ahead despite signs of trouble. Landsberg categorizes this under several cognitive biases, including continuation bias and overconfidence bias. These patterns explain why experienced pilots make seemingly irrational choices. They plan the flight and then feel compelled to stick with it, even when the conditions change dramatically.
Learning From Icarus also highlights another root cause. And that is arrogance. Ignorance can be corrected through education, but arrogance is a personality flaw that resists feedback. For example, when pilots ignore ATC guidance, skip checklists, or show contempt for safety briefings, they put everyone at risk. In one hangar story in the book, a newly rated pilot alienated every instructor at two small airports with his reckless behavior, only to crash a year later while flying VFR into IMC.
The tragedy is not just that human error persists. It’s that the lessons to prevent it have already been written, analyzed, and published in NTSB reports, FAA bulletins, industry publications/videos and in this very book. The tools to avoid disaster are known. The question is whether pilots will be humble enough to adopt them?
